The IAP is the only way a former student may pursue a claim of sexual or serious physical abuse, or other wrongful acts, unless you have opted out of the Settlement Agreement. The Advocacy and Public Information Program provides funding to national, regional and community-based organizations to advocate and provide public education on the components and impacts of the Indian Residential Schools Settlement Agreement.

If you are a former student who lived at an eligible Indian Residential School and were alive on May 30, 2005, you may apply for a Common Experience Payment using this form.
If you are the legal Personal Representative of a mentally incompetent or minor person, or if you are applying for a person who died on or after May 30, 2005 or for someone who attended the Mohawk Institute and who died on or after October 5, 1996, use this form (if you are applying for yourself, please use the "CEP Application Forms" above).
The Guarantor Declaration is to be submitted when the Common Experience Payment (CEP) applicant cannot obtain the documents outlined in the CEP application to support a change of name. In such cases, a Guarantor Declaration is used to attest to the current name being used by the applicant on the CEP application. Service Canada may contact the persons identified in this form to verify their declaration.
If you are not happy with the years for which you were compensated or the amount that you were paid, you may file for Reconsideration. You do not have to provide additional information for your file to be reconsidered, but it may be helpful. This is done by mail, fax, e-mail or telephone. You may use this form.
In order to appeal the decision on your Common Experience Payment, you must first go through the Reconsideration process. An Appeal Form will be mailed to you in the Reconsideration process.
